New Patients
Please arrive 10 minutes early to fill out paperwork (or complete it online in advance — we'll email you a link). Bring a photo ID, your insurance card, and a list of any medications you take.
Insurance & Financing
We accept most major PPO insurance plans and are happy to file claims on your behalf. Not insured? We offer an in-house membership plan and flexible financing through CareCredit.

Policies
We ask for at least 24 hours' notice if you need to reschedule so we can offer your time to another patient. Dental emergencies are always seen the same day when possible — call us right away.
